To make an appointment with any of our health care providers, please do one of the following:
Visit our Patient Portal and create an account.
New patients will be able to complete all of the new patient paperwork and request a preferred day and time for the appointment. The New Patient Coordinator will contact you to finalize scheduling the appointment. Existing patients may request a preferred day and time for the appointment as well, and a Front Office Secretary will contact you to finalize scheduling the appointment.
New Patients, it is the responsibility of the patient to obtain medical records from the referring provider. Please request only RECENT (last visit) office notes, laboratory results, EKG, or any other recent test that pertaining to your visit.

Our offices are open Monday through Friday; our phone hours are 8:30am to 12:00pm and 1:30 – 4:00pm.

We ask that our new patients arrive 30 minutes prior to their scheduled appointment time and follow up appointments arrive 15 minutes prior to their scheduled appointment time.
If you are unable to keep your appointment, please notify our staff as soon as possible. We will do our best to reschedule your appointment for the next available opening.
